A Biometric Screening is a short health examination that determines your risk level for certain diseases and medical conditions. It includes: 

  • a check of your blood pressure, height, weight, and body mass index by your health care provider 
  • a blood test that includes fasting or non-fasting blood sugar and cholesterol. 

It gives you important baseline information about your general health and helps you focus on actions to maintain or improve your health.

  • If you have completed a Biometric Screening since June 1, 2013 there is no need to complete it again. All you need to do is have your doctor complete the Biometric Health Screening Form and mail, fax, or email it to the location listed on the form. 
  • If you have not completed a Biometric Screening since June 1, 2013, visit your doctor for the screening anytime from June 1-August 31, 2014, then have your doctor complete the Biometric Health Screening Form and send it to the location listed on the form.

Once you and your doctor have completed the Biometric Health Screening Confirmation Form, send it to the location listed on the form. Your physician may designate completion of the form to the medical staff who are under his direction.

On-site screenings: If you cannot go to your doctor’s office, there will be a limited number of on-site Biometric Screenings available at various County work sites. Your physician is not required to sign the form if you received your screening at an onsite location but you should keep a copy of your screening results for your records.
