Keep a copy of your completed Biometric Health Screening Confirmation Form and your HRA print out for your records. You should discuss your Biometric Screening results (Know Your Numbers!) and your HRA results (Know Your Health Risks) with your doctor to determine next steps toward a healthier tomorrow. The Biometric Health Screening Confirmation Form will be available on this website prior to June 1, 2014.
After your Biometric Screening – allow 4-6 weeks for processing Once you and your doctor have completed the Biometric Health Screening Confirmation Form, send it to the location listed on the form. Your physician may delegate completion of the form to the medical staff who are under his or her direction.
If you obtained your screening at one of the various County onsite screening locations, your physician is not required to complete the Biometric Health Screening Confirmation Form, but you should keep a copy of your screening results for your records and to discuss with your physician, if needed.
Once you submit your completed form or attend on onsite screening Summit Health will notify your health plan that you completed it. From June through August, your health plan will notify the County of Orange Benefits Center of employees who have completed the Biometric Screening.

You complete your HRA online through your health plan’s website. From June through August, your health plan will notify the County of Orange Benefits Center of employees who have completed the HRA. Non-smoking attestation – To be done during Open Enrollment, November 1-21, 2014
Once Open Enrollment begins in November, you can log on to the Benefits Center Website and complete the non-smoking attestation. If you are currently a smoker but would like quit, talk to your doctor and/or enroll in a smoking cessation program. There are many low or no cost options available. Go to Step 3 for more information.
